Trying to work out how a non-rager could have a rager class 3 reaction (abilities go into hyperdrive).
Premis is an Energizer/regen/Healer (complicated, but I can string it together in a plausible way) is put in a position where they are pushing their abilities to the limit. I was thinking burnout or an outside power source which either burns out the healing completely or leaves it on full time which is a drain on the kid's energy stores.

I don't see why you couldn't do such a thing with a straight up energizer. Instead of just having a straight up pipeline to their 'outside' power source as is standard, they have a slightly more complicated staged/turbo set up. Under normal conditions, the power flows through the main line on a normal draw. In special cases, the power it tapped into a side system which creates a sort of 'turbo charging' of the normal line, that instead of just letting it flow, its actively drawing it in allowing the abilities to more aggressive use/performance. And this could work on several tiers, with the upper tiers of the power being able to draw more than is typically safe... which would result in a burn-out like condition. And, of course, there's nothing stopping you from tying this to rage/aggression or whatever else you want to... it probably would become increasingly uncomfortable to use (especially if upper tiers actually damage the body instead of just straining it beyond normal comfortable levels)... and its not uncommon to need strong emotional context to work through that sort of discomfort or pain.

You can give the character a psychological or emotional block that keeps them from using all the power they are capable of. But if they get sufficiently angry they forget all about those hangups and go all out until they run out of stream. Or consciousness.
Then they come to their senses and are properly horrified of the damaged they did, which then reinforces the already existing block even more.
Ito Sensei would have a field day with this, but may not be able to do anything about it without doing more mental damage to the poor mutant who is increasingly convinced he or she is a ultra violent rager.
